Pathways to Success (P2S)

At P2S, we are dedicated to supporting youth in achieving their vision of a successful future. Our program is specially tailored for at-risk youth aged 14-25 from underserved communities throughout Los Angeles County. We focus particularly on those who have faced adverse childhood experiences (ACEs), offering a comprehensive range of wraparound services aimed at fostering lifelong health and wellbeing.
 

“Pathways to Success (P2S)” youth initiative is meticulously designed to empower youth and their families from underserved communities.  We accomplish this through pioneering resource programs, nutritional guidance, and physical activity plans, supplemented with instructive workshops and supportive mentorships.  Our foremost priority is to provide a nurturing, secure haven for these families dealing with adversity.

Transition to Independence Process (TIP)

  • Who are we?
    Lifedriven Foundation, Inc. (LFI), a 501 c (3) was established in September 2006, is deeply committed to supporting cancer research, care prevention, ensuring access to resources, information for cancer prevention, awareness, screenings and helping children transition from patients to joyful kids once again. LFI’s mission is to provide solutions to social problems that help to enhance the lives and circumstances of children in disadvantaged communities.

  • What is Transition to Independence Process (TIP)?
    TIP is a young adult mental health program designed to address the unmet needs of young people in our communities. Navigating life's challenges independently can often be overwhelming, but TIP steps in to bridge those gaps for the youth in our communities.
  • How does the program work?
    The program is driven by the voice of the individual in the program. Each youth will create a “Future Plan” with their assigned TIP Program Facilitator. The Future Plan then becomes a roadmap to a successful, independent future. At the end of each session, the Future Plan is revised and updated to ensure it continues to guide them towards independence after they have completed the program.
  • How long is the program?
    Our programs typically run for 5-week sessions, held exclusively on Saturdays. However, this duration can be adjusted based on the level of involvement we receive. Several factors are taken into consideration when planning the sessions, including scheduling, staff availability, group events, and individual goals. We encourage you to discuss your specific needs with your Program facilitator to ensure the best possible outcome.
  • Does P2S provide transportation and/or housing?
    P2S does not directly provide housing; however, we can help you connect with the right resources in your county. While we do provide transportation, it is at the discretion of our Program Director. We encourage you to discuss a transportation plan with your program facilitator. Transportation to group events will be provided if needed.
  • Do my parents or other professionals need to be involved?
    If you are a minor, we will require a parent, or legal guardian written permission to attend the program. P2S is a person-centered program which means that everyone takes ownership of their own path. While we do not require parents or other professionals to be involved, we encourage all participants in the program to identify people that are helpful and supportive in their life.
  • What are group events / speaker series? Are they mandatory?
    Group events are a good opportunity to connect with peers, use community resources and develop new skills to meet goals. These events are organized by our team staff to include both recreational and skill building groups. No one is required to attend, and your facilitator will inform you about upcoming events based on your interest and willingness to participate. We are always open to new ideas so that we can organize events that best meet the need and interest of everyone in our program. Please be encouraged to discuss your idea with us!
  • How do I get started?
